The Aims of the Patient Participation Group
As your GP practice, we are planning to have greater involvement with our patients to give you and your family an opportunity to help develop your practice and set up a patient panel.
The practice is also a member of the Leeds Clinical Commissioning Group which is an NHS organisation made up of local GPs who are responsible for planning health services for people across Leeds. Leeds is also setting up a patient panel to involve people in planning local health services.
For both groups we would like local people to share their ideas and opinions. This could be done through occasionally responding to surveys, questionnaires, by taking part in a discussion group every now and again or by joining a patient group which meets every few months or taking part in an online group.

Getting Your View
The group would like to contact patients on occasion by email and/or text so that they can obtain the views of the widest group of patients possible. We would like to obtain your email address and mobile phone number to do this.
