New Patient Registration
If you reside within the practice boundary you may apply to be registered at the Practice, subject to the Practice having an open list.
To register with the Practice you will need to complete a Patient Registration Questionnaire with basic medical details as sometimes records from your previous practice take a while to arrive, a GMS1 form and Summary of Care Record (you can print out the forms below) please complete as fully as you are able to and bring to the Practice along with two forms of identification, such as a passport or driving licence (photo ID) and a recent utility bill or bank statement. Alternatively you can email these forms to us but you will still need to call at the Practice to sign the forms and produce your identification before you are registered. Proof of identity will be verified by a member of staff.
Please note all children aged 16 years and under must attend at the Practice for a New Patient Registration Check with our Health Care Assistant. All new patients have the option to attend for a New Patient Registration Check, but as stated above it is compulsory for all patients 16 years and under.
